Description

Medical

Medicare-eligible: Medicare Supplement medical plan; payment of Medicare copayments and deductibles after a $250 per person annual deductible is met

You must enroll in Medicare Part A and Part B or no coverage is provided by the Welfare Fund.

Non Medicare-eligible: Preferred Provider Organization (PPO) plan, including inpatient hospital benefits, administered by Independence Blue Cross

Vision You may receive up to $300 per person every two years for an exam, lenses, frames, and contact lenses.
Hearing Aid You may receive 100% of the cost of a hearing aid, up to a maximum $1,000 payment per device (maximum of 4 devices per lifetime)
Dental Dental coverage through Fidelio Insurance Company. After you meet the $50 family deductible, Fidelio pays 100% of usual, customary, and reasonable (UCR) charges for all eligible dental expenses to a maximum of $750 annually per family.
Prescription Drug You pay $10 for generic, $20 for preferred brand, and $40 for for non-preferred brand at retail pharmacy. Mail order is two times the retail copay.
Employee assistance Program (EAP) Free short-term counseling over the phone, 24 hours a day, seven days a week. The Welfare Fund also pays the full cost for up to three face-to face visits with a trained mental health professional.
Mental Health and Substance Abuse

Medicare-eligiible: Medicare Supplement plan; payment of Medicare copayments and deductibles. The $250 per person annual deductible can be met here, or in the Medical Plan, or in combination of both.

Non Medicare-eligible: Coverage for all or a portion of your inpatient and/or outpatient care. All treatment must be precertified or benefits will not be paid. 

Death Benefit If you retire on a Disability Pension, or if your age plus pension credits total at least 80 when you retire, a $5,000 death benefit is paid to your beneficiary. This benefit is payable through the Pension Fund. Call the Fund Office or see the Pension Plan SPD for details.
