The IUOE Welfare Fund of Eastern Pennsylvania and Delaware offers several plan options to our members.
Inpatient hospital and medical/surgical benefits offered to our eligible participants are administered by Independence Blue Cross Personal Choice, a Preferred Provider Organization.

Mental health and substance abuse benefits are administered by PCN.

Exclusions from our medical coverage include:

• Motor Vehicle Accident  - Services and supplies for injuries resulting from a motor vehicle accident are not covered.
• Infertility Treatment - Charges for the testing or treatment of infertility, including drugs, in vitro fertilization, artificial insemination, and the reversal of vasectomy or tubal ligation.
• Maternity Benefits  - Services and supplies for anyone except the member or spouse of the member.
• Cosmetic Surgery - Cosmetic or reconstructive surgery or treatment, except due to accidental injury or correction of a congenital anomaly.
• Workers' Compensation - Medical and hospital services and supplies for injuries or sickness that are covered under a workers' compensation act or similar law.
  

 

Claim Submissions

MEDICAL CLAIMS

All non-Medicare medical/surgical claims must be billed directly to Independence Blue Cross, 1901 Market Street, Philadelphia, PA 19103.

DENTAL CLAIMS

Dental claims should be submitted directly to Fidelio Insurance Company, 2826 Mt. Carmel Avenue, Glenside, PA 19038.

VISION CLAIMS

Vision claims should be submitted directly to Vision Benefits of America.  You may contact them by phone 1-800-432-4966 or through their website www.visionbenefits.com .
